1 serving of chicken = 4 oz.

One serving of chicken is 4 oz., not 1 oz. You'd get pretty sick over time if you only considered one serving of any meat to be 1 oz. You wouldn't get enough protein or other nutrients.

anyone know how we can edit the food groups on this should be four servings of meat, not one, since it's listed as four ounces.

That is an impossibility for a baked lean breast with nothing added pre/post cooking.

Is this cooked or raw?

This is the raw weight information. MattK's comment from last year quotes the cooked weight. So be careful when using this number. 4 oz raw is roughly 3 oz cooked.
